COVID-19 Exemptions

If you are subject to mandatory vaccination and are requesting an exemption from the vaccination requirement, you must submit either:
Completed Medical Exemption Forms must be submitted to access@hpu.edu
Completed Religious Exemption Forms must be submitted to registrar@hpu.edu
Once your request for accommodation has been reviewed and processed, you will be notified if an exemption has been granted or denied. If approved, you will be notified as to the additional requirements with which you must comply, as may be updated from time to time. Once granted, a religious exemption is valid for the current academic year.
If an exemption is approved, you will still be required to comply with University safety protocols, rules, policies and procedures as well as the additional requirements for unvaccinated students, including mandatory testing. In addition, the University may from time to time establish additional rules to protect the public health and safety or in response to applicable local or state orders. These rules may limit your continued participation in campus activities, events or programs.
Please be aware that certain programs, such as clinical practicum programs or others, may impose different vaccination requirements for participation, access or attendance and that this exemption may not satisfy the vaccination requirements of third parties.
